2. You must submit a product review for the above product via your PriceGrabber user account.
3. Only UPC codes from the retail box will be accepted. Refurbished, open-box, white-box, OEM, and generic items are excluded from this offer.
4. Failure to comply with all requirements specified in this bonus form shall render you ineligible for a bonus. Illegible, fraudulent and incomplete bonus forms shall be deemed invalid.
5. This non-transferable bonus form is only available to the first 50 individuals that request this form.
6. Bonus eligibility is limited to one (1) per person, per email, per address.
7. You are allowed a total maximum of 5 submissions (for different products)per calendar month (based on the postmark date) under this program. Without limitation, we consider the same email or same address as one person.
