Description: To receive your Yamakawa USA, Inc. rebate returned by mail, follow these instructions: 1) Yamakawa is not responsible for lost, stolen or misdirected mail. 2) Limit one rebate per name, receipt, family, address and household. 3) To receive your $50 rebate check, you must mail the following items: A: Rebate form completely filled out including signature. B: Original UPC Bar code label cut out from box. C: E-COST receipt/packing list with purchase price circled and date of purchase clearly visible. (Please make copies of all submitted items for your records.) Order confirmation e-mails are not considered acceptable receipts. 4) Incomplete coupons will not be accepted. Coupons must have the serial number and customers signature. 5) Clearly handprint information on rebate coupon. 6) Original sales receipts must be dated no earlier than November1st 2004 and no later than February 28th, 2005. Mail items A, B, & C in a single envelope to this address so that your submission is postmarked within 14 days of the purchase date that appears on your receipt. Submissions postmarked later than 14 days after the purchase date will not qualify. 7) Faxed copies or emails will not be accepted. 8) Fraudulent submissions of multiple requests or falsification of information will void this offer and could result in federal prosecution under the U.S. Mail Fraud Statutes (18 USC Section 1341 & 1342). 9) Offer only valid to E-COST end user customers. This offer is not valid to resellers, retailers or distributors of YAMAKAWA products and their families. 10) Allow 10-12 weeks after claim is received by Fulfillment Company to receive your rebate. Requests from P.O. Boxes are not accepted! 11) All rebates will be paid in U.S. Dollars and is valid in the United States only 12) All materials submitted will become property of the fulfillment company. 13) Envelope should not be larger than 4" x 9 ½". Requests not containing the required information will not be honored or returned. 14) Yamakawa product cannot be returned once the rebate form has been submitted. No resubmissions allowed.
