If you wish to mail your rebate: for purchases made at Staples Stores, send this completed ORIGINAL rebate redemption form printed from the cash register. With the cash register form, no UPC label or additional sales receipt is required. If you are using any other type of Staples Rebate Redemption Form, you MUST attach your ORIGINAL Sales or Rebate Receipt. No UPC label is required. For purchases made at Staples.com: Send this ORIGINAL completed rebate redemption form with the ORIGINAL packing slip that is enclosed with your purchase as your receipt. Photocopies will not be accepted. No UPC Label is required. Offer not valid with any other rebates, offers or promotions. Offer valid in US only.
