To claim and receive your rebate by mail:
Purchase ONE (1) 3-IN-ONE multi-purpose oil (3oz, 4oz, or 8oz sizes) product from any retailer.
Complete and print this original rebate form - no reproductions accepted.
Include your original sales receipts. Sales receipts must clearly show store names, store addresses, products (and their prices) purchased and purchase dates. (Make a photocopy of receipts to keep for your records.)
